The Diocese of Nashville has developed the Safe Environment Program to comply with the the United States Catholic Conference of Bishops. They adopted a charter that ensures that children and youth who worship, study, or participate in activites can do so in the safest and most secure environment possible. The Diocese requires all volunteers, faculty and staff of the Church and Academy to complete the appropriate forms prior to participating in parish or school activities involving minors. We sincerely appreciate all your efforts made on behalf of our children and their safety. Please print and complete Profile Form A if you are a first time volunteer. If you are a returning volunteer and have previously completed a packet, please complete the Form B/Training form (we require a new one every year). Hermitage Learning Center
Hermitage Learning Center now offers in-school tutoring for Grades K - 8. Please see the information letter for parents in the documents list on the right.
Children with chronic medical conditions that require regular medication will need to have a form on file in the school clinic signed by the child's physician and parent. Medication must be turned in to the clinic in the original container with the prescription attached along with the prescription medication form that can be found to the left under documents. New forms must be submitted yearly.
